  3. Submit a Reimbursement Request – Send your receipt and prescription to your healthcare provider along with any required forms.

  4. Receive Your Reimbursement – Your provider will review your request and determine what percentage they will cover.

Since every insurance plan is different, we recommend checking with your provider beforehand to confirm eligibility and requirements.
